Refusing to take a urine test is considered "disobeying a direct order". This is viewed as "major" because without some sort of order in the prison, the staff would totally lose control. The punishment is as severe as it can be to hold out as an example to others thinking about UA refusal. He will get placed in Disciplinary Segregation, also known as the SHU, the Special Housing Unit, solitary confinement, or the "hole". There, he will be in a cell 24/7 except for one hour for a shower, three times a week, two hours a week recreation (which consists of a cage-like structure on asphalt with nothing more than possibly dip bars to do exercise and get some fresh air. This segregation could be for days, weeks or months. He could lose visitation, phone and commissary privileges as well, it all depends on the officer writing up the incident report. If there were other factions that exasperated the violation, he could get it all against him. In the worst case scenario, they could transfer him to a higher custody prison for the balance of his sentence after serving 6-12 months in isolation.
