You can try, and the fact that you are willing to is meaningful. But the honest answer is that if he does not want help, there is very little you or anyone else can do to make treatment happen. Mental health treatment in the prison system, like outside of it, requires the person to voluntarily engage. Someone who is not ready to acknowledge they need help or does not want to participate cannot be forced into a treatment program in most circumstances.

What you can do from the outside is keep the lines of communication open through letters and calls, and over time, plant the idea that treatment is available and that seeking it is not a weakness. Sometimes that kind of consistent, patient presence from someone who cares is what eventually tips the scale when the person is ready. There is no timeline on that and no guarantee, but relationships matter in ways that are hard to quantify.

You can also contact the facility's mental health department or psychology department to flag your concern. As a friend rather than a legal representative or family member, they are limited in what they can share with you, but raising the concern formally puts it on record. In cases where an inmate's mental health deteriorates to the point where they pose a risk to themselves or others, the institution has its own obligations to intervene regardless of the inmate's wishes.

The path forward is patience and consistency. You cannot do it for him, but you can stay in his corner.
